Please fill out the form below in preparation for your upcoming appointment. If you have any questions while filling out the form, call the office at 802-662-5966. On the day of your appointment please arrive 5-10 minutes prior to your appointment time. Call the office once you arrive and a member of the team will meet you in the entryway to complete your temperature screening. Please come into the office wearing a mask.
